Your role in the Homme Youth & Family Programs referral process is critical. 

We'll do all we can to ensure that your referral is treated with dignity, confidentiality and respect. That's part of our philosophy: to treat each youth as a unique individual.

2. Submit all relevant written material to the Admissions
     Department. 
3. A pre-placement interview is conducted to determine the  
    adolescent's acuity level, strengths and needs. 
4. Aid in identifying and coordinating with collateral contacts in  
     previous placements and community resources. 
5. Aid in completion of an intake packet. 
6. Coordinate placement date and transportation.

 
Before beginning the admissions process, please be aware that the following documents must be furnished before a child will be admitted: 
 
  • Copy of original birth certificate 
  • Copy of court order to agency 
  • Copy of referring agency's case plan 
  • Current MA or medical & dental insurance card 
  • Copy of Social Security card 
  • Immunization records 
  • Social history 
  • Insurance forms (if applicable) 
  • Copy of physical & dental examinations done in the past 12 months
  • 30-day supply of ALL prescribed medication (if applicable)
  • Type 2 forms (if applicable)
