Admissions Criteria:

Males and females between the ages of 10 and 20 (18 to 20 on a juvenile order) are eligible for placement with Homme. Each referral is reviewed and a face to face admissions screen is held. Homme is committed to quality care and wishes to be certain that the individual is matched to the appropriate program, which can best assist him/her.

We will do all we can to ensure that your referral is treated with dignity, confidentiality and respect. That’s part of our philosophy – to treat each youth as a unique individual.

 

Placement Options:  Homme has offered treatment-specific programming for males and females since 1985 when it opened its first sex offender program.  Programs are now available for sexually acting out youth, abuse victims, fire-starters, cognitively delayed and community-based programming.

Assessment:  Homme will do 30-day placements to determine treatment needs, stabilize behaviors and assess risk as well as appropriate intervention.

Short-term:  Homme will collaborate with other agencies to provide short-term care to youth that are placed while community-based programming is arranged.  We have 60-day and 90-day programming to be used in conjunction with intensive aftercare.

Graduation:  Homme continues to serve youth long-term. These youth complete a multi-tiered level system, which takes them from assessment to relapse prevention through an objective-based program.

Affiliations and Accreditations:  Accredited by Council on Accreditation (COA), Member of Wisconsin Association of Family & Children’s Agencies (WAFCA), Licensed as a CCI by State of Wisconsin Department of Health and Family Service, Licensed as a Type 2 Facility by the State of Wisconsin, Certified by the Minnesota Department of Corrections, Approved by the State of Michigan Family Independence Agency (FIA), approved by Illinois Governor’s Purchase of Care Review Board, and licensed as a CBRF by the State of Wisconsin.

  1. Submit all relevant written material to Admissions Department.
  2. A pre-placement interview is conducted to determine the adolescent’s acuity level, strengths and needs of the youth and family.
  3. Aid in identifying and coordinating with collateral contacts in previous placements and community resources.
  4. Aid in completion of intake packet.
  5. Coordinate placement date and transportation.

Please be aware that the following documents must be furnished for placement.

 

  • Copy of original birth certificate

  • Copy of court order to agency

  • Copy of referring agency’s case plan

  • Current MA or medical & dental insurance card

  • Copy of social security card

  • Immunization records

  • Social history and individualized education plan

  • Insurance forms (if applicable)

  • Copy of physical and dental examinations done in the past 12 months

  • 30-day supply of ALL prescribed medication (if applicable)

  • Type 2 forms (if applicable)
